Background

A SPECIAL NEEDS PLAN FOR THE MOST VULNERABLE

Provider Partners Health Plans offers Medicare Advantage Institutional Special Needs Plans (I-SNP and IE-SNP) designed to improve care for Medicare’s beneficiaries who reside in long-term care facilities or assisted living facilities who need the type of care that is provided in a nursing home. Provider Partners is managed by a group of highly experienced health care leaders that work with physicians and other providers to improve care and outcomes for members.

The plans replace traditional Medicare coverage with an all-in-one solution that covers:

Inpatient care (Part A)

Outpatient and primary care (Part B)

Prescription drugs (Part D)
