Provider Partners Health Plans offers Medicare Advantage Institutional Special Needs Plans (I-SNP and IE-SNP) designed to improve care for Medicare’s beneficiaries who reside in long-term care facilities or assisted living facilities who need the type of care that is provided in a nursing home.
